Group Memberships – Colorado Speech-Language-Hearing Association

Group Memberships are available when people from the same workplace join CSHA. Discounts are available based on the number of members.

6-10 Members = 20% discount per member
11+ Members = 25% discount per member

Printable Group Membership Form Employer must complete this form and include it with all individual printable membership applications. All documents can be mailed or faxed with a check or credit card for payment.
Online Group Membership Form Complete this online form and include scanned membership applications. This online version is available with credit card payment only.
